Terms & Conditions for Empire State Pizza Kids Reading Program:Eligibility: This program is open to children ages 16 and under. Each child can participate in the program once per month by reading five (5) books within a calendar month.Program Participation: To claim a free personal cheese pizza, the child must read five (5) books in one calendar month. After completing the books, a form must be filled out either online or in-store, with assistance from a parent, guardian, or teacher. The form must include all requested details to validate the completion of the reading requirement.Free Pizza Redemption: Upon successful submission of an online form, a voucher for a free personal cheese pizza will be sent via text message or email.Limitations: Each child may only earn one free personal cheese pizza per month, regardless of the number of books read beyond the five required. Vouchers are non-transferable and may only be used by the child who earned them.Voucher Validity: Vouchers are valid for 30 days from the date of issue. Expired vouchers cannot be reissued.Program Availability: Empire State Pizza reserves the right to modify or terminate the Kids Reading Program at any time without prior notice.Parental/Guardian Consent: By submitting the form, a parent, guardian, or teacher consents to their child’s participation and agrees to provide accurate and truthful information. Empire State Pizza reserves the right to disqualify any submissions that are deemed fraudulent or incomplete.Privacy: All personal information collected as part of this program will be used solely for the purpose of administering the reading program and issuing vouchers. Empire State Pizza does not sell or share this information with third parties. By participating in this program, you agree to receive text and email marketing notifications.
